Welcome to Coordinated School Health at Jonesborough Middle School. My name is Carrie Rogers, and I am the school nurse. My contact information is email@example.com or 423-753-1195. Please feel free to contact me with any questions, concerns, or information that will help us take care of your child during school hours. Here are some reminders, guidelines for the clinic.
Clinic Hours are 7:35 to 3:05 Mon-Fri.
Students cannot carry any medication on their person to and from school. This icludes cough drops, eye drops etc. All these medications must be brought to the school nurse by a parent or guardian.
Please let us know of any allergies your child may have.
Reminder to all sixth graders: Prior to entering 7th grade you must have documented proof on the TN Immunization Form that you have had a Tdap booster and a 2nd Varicella(chicken pox). Students who do not have this, medical exemption, religious exemption, or appt card will be sent home from school beginning the first day of school. Any absences will be unexcused.
The school does not supply any medication. Any medication your child takes during school hours must be brought in by a parent/guardian and an instruction/permission form must be filled out. This includes any over the counter meds such as Tylenol,Ibuprofen, cough syrup, cough drops, etc.
FOOD BAGS FOR KIDS: This program is sponsored by The Second Harvest Food Bank and is available to any student their siblings. The deliveries are every two weeks and are sent home with the student from school. If you feel you could benefit from this service please call me or the School Guidance Counselor.
Volunteers are always needed. We have a Healthy School Team here at school that consists of Teachers, Students, Adm. Faculty, Food service, parents, and community members. If you are willing to help with activities planned by the Healthy School Team please let me know. We appreciate any and all assistance.
