Kindergarten Registration for Children Entering Fall of 2016
When: Wednesday, July 20, 2016, between the hours of 8:30 am and 3:00 am Call 434-4923 or email firstname.lastname@example.org for an appointment.
Where: Asbury Family Resource Center, 2002 Indian Ridge Road, Johnson City, TN 37604
Bring: Child's official birth certificate, social security card, Tennessee immunization certificate/physical form, and proof of residence address (e.g., utility bill)
Seventh Grade Immunization Requirements
If your child will be entering 7th grade in the 2016-2017 school year, you will receive parent notification of required immunizations. Your child will need both a DTap (Tetanus-diphtheria-pertussis) booster and verification of immunity to Varicella (chickenpox) – two doses or date of disease.
Your child will need proof (official TN Immunization Record, either from the Health Department or your child's physician) of immunizations by the first day of school. Seventh grade students who do not have the required immunizations documented on the TN Immunization Record will be excluded until proof is given to the school.

Office of Coordinated School Health
Washington County Health Department in collaboration with the Johnson City and Washington County School Systems is offering a late afternoon immunization clinic by appointment only for children in the 6th – 12th grades. This clinic is to assist in better preparing children entering the 7th grade and High School adolescents who plan on attending college in need of required immunizations such as the Tdap, Varicella (Chicken pox), MMR, Meningococcal, and other vaccines parents/guardians may request.
