CONFERENCE FORMS
can be used to document many types of meetings - with parents, teachers, and student.
- A conference form is not necessary during an IEP meeting unless discussion takes place that requires more in-depth explanation. You may fill out a Conference Form documenting what was discussed and have meeting participants sign. Be sure to make a copy for parent. You DO NOT have to do a Conference Form at every IEP meeting.
- If parents request an impromptu meeting, use a Conference Form to document meeting discussion.
- When you meet with student/teachers to discuss behavior issues and devise an intervention, a conference form can serve as a behavior contract with a student when behaviors are not at a level warranting a Behavior Intervention Plan.
Basically, use a Conference Form to document any type of meeting on or with a student in which an IEP is not required.
